First: Blue Shield of California Is Not Anthem Blue Cross

California is one of the few states with two separate Blue Cross Blue Shield licensees, and people mix them up constantly. Blue Shield of California is a nonprofit health plan headquartered in Oakland. Anthem Blue Cross of California is a different company, part of Elevance Health. They have different networks, different member portals, different phone numbers, and different behavioral health rules.

This matters more for mental health than for most care, because behavioral health networks are narrower than medical networks to begin with. A therapist who takes Anthem Blue Cross is not automatically available to a Blue Shield member, and searching the wrong company's directory is one of the more common reasons a search stalls before it starts.

What Changed in 2026: Blue Shield Now Runs Behavioral Health Itself

As of January 1, 2026, Blue Shield of California manages behavioral health benefits directly for all of its members. Before that, mental health and substance use benefits were administered by a separate Mental Health Service Administrator that maintained its own clinician network and its own authorization process. Blue Shield brought that function in-house to put behavioral health on the same footing as the rest of the plan.

For most members the change is invisible. It matters if your therapist was in the old administrator's network and did not continue into the new one. Blue Shield notified affected members, and California continuity of care rules can let you keep seeing your existing clinician for a defined period while you transition, particularly if you are in active treatment for an ongoing condition. If you got one of those letters and did nothing, it is worth calling the mental health number on your card now rather than discovering the gap at your next claim.

One practical consequence: if you looked for a Blue Shield therapist before 2026 and gave up, the network you searched is not the network that exists today. It is worth a fresh look.

How Coverage Varies by Blue Shield Plan Type

Blue Shield sells several plan designs in California, and the plan type - not the metal tier - is what determines your referral rules and your out-of-network options.

Trio HMO

Blue Shield's narrow-network HMO, offered in a subset of California counties and common on Covered California plans. Lowest premiums, smallest clinician list, and care is coordinated through your medical group. No out-of-network benefits except emergencies.

Access+ HMO

A broader HMO network than Trio, common in employer and public-sector plans. Still HMO rules, so your care runs through a medical group, but with a wider list of clinicians and hospitals to choose from.

Full PPO

Blue Shield's largest network and the most flexible option. You can self-refer to any in-network behavioral health clinician, and you have out-of-network benefits if you are willing to pay a higher share.

Tandem PPO

A subset of the Full PPO network at a lower premium, with the same plan benefits. The catch is the smaller network, so a clinician who takes Full PPO is not guaranteed to take Tandem. Confirm the specific network name on your card, not just "Blue Shield PPO."

Blue Shield Promise (Medi-Cal)

Blue Shield's Medi-Cal managed care plan. Medi-Cal behavioral health works differently from commercial coverage - mild to moderate conditions run through the health plan, while more severe conditions run through the county mental health plan, so the first question is which side of that line your care falls on.

Call the mental health number on the back of your member ID card - it is often a different number from general member services, and since 2026 it reaches Blue Shield directly rather than a separate administrator. Ask these five things and write down the answers:

  • What is my copay or coinsurance for an outpatient behavioral health visit (CPT 90837)?
  • Does my deductible apply to behavioral health, and how much of it is left this year?
  • Do I need a referral or prior authorization for outpatient therapy?
  • What is my exact network name - Trio, Access+, Full PPO, or Tandem?
  • Do I have out-of-network behavioral health benefits, and at what percentage?

If Blue Shield Denies Your Care

A denial is not the end of the process, and California members have unusually strong appeal rights. After the plan's internal appeal, you can request an Independent Medical Review through the Department of Managed Health Care at no cost to you, and an outside clinician reviews the decision.
